The Owner agrees the following:
I certify that I am the owner of dog or dogs to be trained and I give permission to The Harvard of Dog Training® and the
trainer to use the methods that they see fit and are approved by me to help me in the training. I understand that the
methods used by the trainers are the most effective and humane methods available and that they are professionals and
will know exactly what my dog needs in order for training to be successful. I understand and agree that The Harvard of
Dog Training® and its employees shall not be liable for any injury or damage to any person, animal or property which
results from the training or behavior of my pet/dog. My dog must also be healthy and free from any and all communicable
diseases and conditions upon starting the training and I must inform the trainer if my dog gets sick, or has any health
issues during the duration of the program.I understand that for the safety of all pets/dogs, my dog(s) must be current on
all vaccinations I realize that the training that is offered to me, my dog and my family by The Harvard of Dog Training® is
the best available and that any and all advice given to me by the trainer or representative of The Harvard of Dog
Training® is done in good faith and to help me with the training process of my dog. I understand that any and all trainers,
evaluators and office staff are rigorously trained and tested by Richard Heinz, The Miami Dog Whisperer® and CEO of
The Harvard of Dog training®. All training and methods are implemented, and are closely monitored and followed by The
Miami Dog Whisperer® and CEO himself. The Harvard of Dog Training® is concerned about your privacy and will not sell
or lease your personal information to any third party. The Harvard of Dog Training® will use your personal information for
the purpose of administering this program only and sending you special promotions and valuable offers from our
company. .I also understand and agree that any child under the age of 16 years must be accompanied by an adult at all
times if participating in the training. I further agree that The Harvard of Dog Training® and its employees shall not be held
liable for any costs or an expense incurred as a result of my pet’s/dog’s participation in the program. Classes are meant to
run consecutively as explained in initial evaluation (typically once per week from 45 minutes to 1 hour depending on
owner, dog and situation), although occasionally due to unforeseen circumstances or acts of nature, a week may be
skipped, rescheduled or canceled. If the class is cancelled by the trainer, the trainer will contact me and schedule a new
time and day. I understand and agree that if I miss, cancel or skip a class, I will be responsible for letting trainer know 24
hours in advanced, and rescheduling that class at the trainer’s discretion. I realize that make up classes’ time and day will
be determined according to availability and are usually rescheduled for the same time and day the following week, unless
discussed otherwise with trainer. If there is a class scheduled, and I am not present at the time and place agreed, or I
forget, and not cancel the class ahead of time and trainer comes to the class and I am not there, I will be charged a $25
penalty, payable to the trainer the following class.I realize that it is extremely difficult to change my day and time with short
notice because The Harvard of Dog Training® will not move or change other previously scheduled clients to make room
for someone who missed or needs to reschedule, therefore it will be done according to availability understand that if there
is a need to put my program on temporary hold, I will have no more than 4 months to continue my classes and I am
responsible for contacting The Harvard of Dog Training® to be put back on the schedule. If more time is needed, I
understand that I must contact the office and request an extension which may or may not be granted, in writing. If the hold
period goes over 4 months and an extension is not granted , the program will be void, regardless of how many training
sessions I have left and if I chose to continue a start-up fee of $300 will be charged to resume classes. I understand that
the company will do all it can to put me back on the schedule as soon as possible, or within 3 weeks of initial contact, after
a temporary hold. I further understand that in order for this program to be successful and for me to get the results I desire,
I must practice daily what I learn in class. I understand that I, or an adult member of the household will be present in any
and all trainings and will then share the information with the rest of the household so everyone is on the same page. I
realize that payment is due in full the first day of the class (unless other arrangements are made in certain programs). I
understand that there will be NO REFUNDS after second training session is completed, for any payment of my dogs
training or any product or purchases made from or recommended by The Harvard of Dog Training®. However I realize that
if my dog needs additional help, additional free classes may be given at the trainer’s discretion. Programs and trainings
are non transferable and I understand that even if I do not complete all my classes with the trainer because my dog was
trained quicker than expected, I will not get any refund of the unused classes, and I cannot, unless approved by trainer
and CEO transfer these classes to another dog or another person either living with me, or at another location. The
Harvard of Dog Training® reserves the right to refuse or terminate training services of any pet/dog at any time If there are
every any questions, comments or concerns about the trainer, training , my dog, or the company, I will be able to either
call or e-mail the office and get a response within 48 hours or less. I realize that these terms and conditions may be
changed, upgraded or improved at any time without notice. I realize that by signing up for any and all programs at The
Harvard of Dog Training® I am getting the best training in South Florida and I am willing and able to follow all instructions
that are discussed at each lesson to assure the success of my dog and myself in the training. I realize that not all dogs are
created equal an that each dog learns at his own pace and at its own time, so I understand that patience and persistence
are part of the whole dog training process and I agree to this, and realize that the trainer of The Harvard of Dog Training
will be my teacher and instructor through the process.
